My wife is a nurse and a certified massage therapist who has a husband with surfing related back injuries that I'll always have issues with. She has always told me to go 20 min ice, 20 min heat, back and forth as many times as it takes. That and Advil and or red wine!. Tiger Balm works great if you get back spasms.
The idea is that the ice gets the swelling down and the heat brings the healing blood flow and relaxes the muscles so they hopefully don't cramp/spasm.
