Default Inducing Flowering SDC

I've had a Super Dwarf Cavendish for almost a year, now. It's over 5 feet tall, but still hasn't started flowering. It's growing indoors, underneath a 1000W Hortilux Blue lamp. It's in a 25 gallon Geopot filled with about half rockwool cubes, and half Hydroton clay pellets. I drench it twice a day; any less does permanent damage. For several weeks, I've been giving it Technaflora's Recipe for Success Bloom nutrients with the first watering of each day. It's had spider mites a few times, but feeding and spraying with Azamax always seemed to clear that up. For the last few days, some outer leaves have been peeling away from the central stem. I would include some photos with this post, but I'm not allowed, apparently. There are some in my gallery.

Any suggestions on how to induce flowering would be helpful, because I'm running out of vertical space. Thanks!

if the p-stem is over 5ft its not a SDC, more likely to be DC.
